Scholarship Requests

1. MYFA is dedicated to providing opportunities to all athletes regardless of ability to pay registration fees. Please check below to request a Financial Aid Scholarship and attach a school lunch program letter or similar form to this registration. Scholarship requests received after 7/1/12 will be subject to the $25 late fee.

4. MYFA proudly salutes our active military by providing military scholarships to all children of active military.

I/we understand it is my/our responsibility to provide accident and health insurance coverage for my/our child, and I/We will be financially responsible for all charges and fees for emergency medical treatment regardless of whether my medical insurance covers such charges. I/We give MYFA permission to post my child photo on the Medina Youth Football website.
